h-FABP The new biomarker
Human h-FABP (heart-type fatty acid binding protein) is a low molecular cytoplasmic protein (14.9 kDa). It is present extensively in the myocardium. The skeletal muscles also contain h-FABP, but the concentration is several fold lower. Thus, h-FABP is highly specific for the myocardium. In healthy individuals the reference value of h-FABP is relatively low (2-5 ng/ml). Immediately after the onset of myocardial ischemia h-FABP is released into the bloodstream. Within 24 hours it is washed out by renal clearance.
 
 
The traditional markers like Troponin and CK-MB are elevated not before hours after the infarction. In contrast h-FABP can be detected very soon after the infarction.
 
 
The diagnostic reliabity of h-FABP for the early detection of AMI has been tested in a number of clinical studies. In these studies h-FABP showed a significantly higher sensitivity than Troponin while the specificity was slightly lower. The overall diagnostic efficacy was significantly higher for h-FABP.
